Job description

Hospice Registered Nurse, RN - Part Time - Weekends only Saturday & Sunday 8am-4:30pm - we offer some great benefits, competitive pay, 401k with 6% match and an incredibly fun & supportive team environment.
The Registered Nurse is responsible for the delivery of patient care including triaging patient needs over the phone, making visits to patients with changing conditions, and performing admissions as necessary.

Ecumen Hospice North Branch is hiring a Registered Nurse providing services in Chisago County and East Central Minnesota. The Ecumen Hospice North Branch team focuses on providing life honoring care for patients north of the Twin Cities metro area - including, but not limited to: Isanti, Chisago and Pine counties. The Ecumen Hospice mission is to provide care that exceeds the expectations of those that find themselves in the later stages of life by providing a level of comfort and quality of life that makes each person feel honored and supported as their life continues to have significance.

Schedule: Part Time - Weekends only 8am-4:30pm Every Saturday & Sunday
Pay: The targeted pay range for this job is $38.50 - $43.50 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.

Essential Job Responsibilities:
1. Receives and responds to phone calls during assigned shifts as well as making visits to patients with changing conditions and completing weekend admissions.
2. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness.
3. Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
5. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Records pain and/or symptom management changes/outcomes as appropriate.
6. Initiates the plan of care and makes necessary revisions as patient status and needs change. Communicates with the physician regarding patient needs and reports changes in the patient's condition; seeks physicians' orders as required.
7. Provides health care instructions to the patient as appropriate per assessment and plan.
Driving Requirements: Local Travel and Driver's License Required
Registered Nurse HSP Minimum Required Qualifications:
• Graduate of an accredited Registered Nursing program
• 1 year of nursing experience in post-acute care setting
• Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
• Valid Drivers License, proof of insurance and satisfactory motor vehicle record
• Ability to communicate effectively both verbal and written
• Proficient with Microsoft Office Suite
• Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.
Preferred Qualifications:
• Bachelor of Science degree in Nursing
• One year nursing experience in Hospice
