Part-Time RN/LPN - 3 PM to 11 PM

Key Responsibilities
  • Carry out professional duties in line with facility policies, standards of practice, the New Hampshire Nurse Practice Act, OBRA, and HIPAA regulations.
  • Work collaboratively with interdisciplinary team members to develop personalized care plans for residents, updating them as needed and participating in care plan meetings.
  • Implement care protocols as required, including fall prevention, safety measures, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain assessment.
  • Evaluate and document any changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team to support informed decision-making.
  • Provide leadership and guidance to direct caregivers, preparing daily assignments and delegating tasks effectively while supervising care through observation and evaluation.
  • Foster positive relationships with team members, residents, and their families to encourage open communication.
  • Administer and document medications and treatments for residents according to professional standards and nursing department policies, ensuring proper inventory management of narcotics.
  • Promote and maintain a positive culture for both team members and residents.
Qualifications
  • A minimum of a Nursing Degree or Diploma from an accredited institution is required.
  • Experience in a long-term care facility and specialized knowledge in elderly care and rehabilitative nursing is preferred.
  • A valid RN or LPN license in the State of New Hampshire is mandatory. Certification in intravenous administration is preferred. Annual CEUs must be maintained, and CPR certification is required.
