What This Role Entails

This role plays a vital part in supporting Medicaid clients receiving in-home services by conducting annual Resident Assessment Instrument (RAI) and Global Health Evaluation (GHE) assessments. These assessments help identify each client's strengths, needs, and overall health status to ensure they receive the appropriate level of care and services.

Through observation, assessment, and collaboration, you will gather and document critical information used by the State of Missouri to determine continued eligibility and reauthorize in-home services. Your work helps ensure clients receive individualized care that supports their highest level of independence, health, and well-being.

Key responsibilities include:

  • Conducting annual RAI and GHE assessments with Medicaid clients receiving in-home services
  • Evaluating functional abilities, health status, and care needs
  • Accurately documenting findings and maintaining compliance with state requirements
  • Collaborating with clients, caregivers, and interdisciplinary teams
  • Supporting the reauthorization process for Medicaid-funded services
  • Monitoring changes in client status and identifying potential care needs
