Position Overview


Journey Nursing Services providesPrivate Duty Nursing, Home Caregiving, and RN Delegationthroughout Snohomish, King, Whatcom, and surrounding counties.


As aPrivate Duty Nurse (RN/LPN), you will provide compassionate, one-on-one skilled nursing care for medically complex pediatric and adult patients across a variety of community-based settings, including:


  • Private Homes
  • Adult Family Homes
  • Assisted Living Communities


You will work closely with patients, families, physicians, and the interdisciplinary care team to deliver individualized, high-quality care while promoting independence, safety, and the highest possible quality of life.

Our Private Duty Nursing team cares formedically complex pediatric and adult patients with a wide range of skilled nursing needs, providing highly individualized, one-on-one care in the home and community.


This role offers the opportunity to build lasting relationships with patients while utilizing advanced clinical skills in a supportive, collaborative environment.


Schedule & Work Environment


We offer flexible scheduling designed to support both patient needs and work-life balance.

Available opportunities may include:

  • Full-time or Part-time positions
  • Day, evening, overnight, and weekend shifts
  • 8-hour and 12-hour shifts
  • Consistent one-on-one patient assignments whenever possible

Essential Responsibilities

  • Provide one-on-one skilled nursing care in community-based settings.
  • Perform ongoing nursing assessments appropriate to your scope of practice.
  • Implement and follow individualized Plans of Care.
  • Administer medications and treatments as ordered.
  • Monitor patient status and report changes in condition.
  • Collaborate with physicians, Registered Nurses, and interdisciplinary healthcare professionals.
  • Educate patients and families regarding treatments, medications, and disease management.
  • Maintain accurate and timely electronic medical record documentation.
  • Ensure compliance with agency policies and state and federal regulations.
  • Participate in quality improvement initiatives.
  • Foster a culture of compassion, professionalism, accountability, and clinical excellence.
  • Perform other duties as assigned.


Skilled Nursing Responsibilities May Include:

  • Tracheostomy care
  • Ventilator management
  • Airway suctioning
  • Enteral feeding (G-tube/J-tube)
  • Total Parenteral Nutrition (TPN)
  • Peritoneal Dialysis (PD)
  • Central line care
  • Medication administration
  • IV therapy (within scope of practice)
  • Wound care
  • Ostomy care
  • Seizure management
  • Other skilled nursing interventions based on individual patient needs


Qualifications


Required

  • Current Washington State RN or LPN license in good standing
  • Current BLS Certification
  • Strong assessment and clinical skills appropriate to your scope of practice
  • Excellent communication and interpersonal skills
  • Compassionate, patient-centered approach to care
  • Valid driver's license and reliable transportation (if assigned to multiple patient locations)


Preferred

  • One (1) year of nursing experience
  • Private Duty Nursing, Home Health, Home Care, or Hospice experience
  • Experience caring for medically complex pediatric and/or adult patients
  • Experience with ventilators, tracheostomies, feeding tubes, peritoneal dialysis, TPN, or other advanced skilled nursing interventions
