The Winchester Post Acute unit provides care for patients transitioning from hospital to home that can provides services such as post-surgical recovery, chronic illness management, and rehabilitation- helping them regain independence to return to the community.
The multidisciplinary team of nurses, therapists, physicians, and support staff collaboratively work together to deliver a personalized care plan, that also supports healing and emotional well-being. The unit also emphasis patient education, family involvement, and continuity of care that makes patients feel seen heard and empowers them in their recovery.

How you'll contribute
A Post Acute LPN who excels in this role:
  • As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
  • Under the direction of an RN or Post Acute Charge Nurse, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.

What we're looking for
Applicants should have a current Tennessee or compact state LPN license and be a graduate from a Practical Nursing program. Additional requirements include:
  • Certifications: AHA Basic Life Support (BLS) within 30 days of hire.
  • Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
  • Experience: One-year previous experience preferred.
  • Licenses: LPN License for the State of Tennessee.
  • Education: Graduate of a Practical Nursing program.
