Job Title: Licensed Practical Nurse-Registered Nurse LPN/RN
Location: Forestview Manor Assisted Living 153 Parade Road, Meredith, NH 03253 Hours: Full Time and Part Time Job Summary:
The Licensed Practical Nurse-Registered Nurse LPN/RN's are responsible for assisting the Resident Care
Director with the overall implementation, delivery and coordination of Resident Care services at the
Community. The nurses will recommend a plan of care for existing residents, coordinate the resident's care,
and document resident files according to state regulations. Will also assist in monitoring the community's
medication management program. Will have some staff supervisory responsibilities within Resident Care
Department and act as Manager on Duty or on-call as assigned by the Director. This position ensures
compliance with all applicable regulations and standards set forth by the State in which Community operates.
Duties and responsibilities include but are not limited to:
โ€ข Maintain current resident care plans on all residents under his/her care. โ€ข Closely mentors, guides and directs nursing staff โ€ข Follow physician's orders โ€ข Transcribe physician's orders promptly and accurately โ€ข Review resident records daily to assure accuracy and completeness. โ€ข Document comprehensive and complete nursing notes. โ€ข Document and report any unusual or significant findings; contact the physician when necessary. โ€ข Administer medications according to procedure. โ€ข Maintain an accurate and safe control of all drugs for which he/she is responsible. โ€ข Initiate any and all emergency procedures as necessary.
Qualifications include:
โ€ข Valid licensure as Licensed Practical Nurse /LPN within state of New Hampshire โ€ข Valid licensure as Registered Nurse /RN within state of New Hampshire โ€ข Must have current CPR certification โ€ข Previous experience preferred but not required โ€ข Computer proficiency โ€ข Strong interpersonal as well as written and verbal communication skills
