We are currently in search of a dedicated Part-Time RN/LPN to join our evening team (3 PM - 11 PM). In this role, you will deliver direct care to our residents in a collaborative environment, ensuring that their needs are prioritized in our Skilled Nursing facility.

Key Responsibilities
  • Carry out professional duties in line with facility policies, state regulations, and standards of practice, including the Nurse Practice Act, OBRA, and HIPAA.
  • Work with interdisciplinary team members to develop personalized care plans for residents, updating them as needed and participating in care plan meetings.
  • Implement care protocols for various needs, including fall prevention, safety, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain management.
  • Assess and document changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team to support informed decision-making.
  • Provide leadership to direct caregivers, preparing daily assignments and delegating tasks effectively while supervising care delivery.
  • Foster positive relationships with team members, residents, and their families to encourage open communication.
  • Administer and document medications and treatments according to professional guidelines and nursing policies, maintaining a secure inventory for narcotics.
  • Promote and maintain a positive culture for both team members and residents.
Qualifications
  • A minimum of a Nursing Degree or Diploma from an accredited institution is required.
  • Experience in long-term care and knowledge of elderly nursing care is preferred.
  • A valid RN or LPN license in the State of New Hampshire is mandatory. Certification in intravenous administration is preferred. Continuing education units (CEUs) must be maintained annually, and CPR certification is required.
