TITLE OF POSITION: REGISTERED NURSE

JOB DESCRIPTION SUMMARY:
The Registered Nurse reports to the Director of Patient Care Services and or Administrator and is responsible for providing home health clinical services.ย  The registered nurse is responsible for quality; adequacy of services provided and may, where required, supervise staff. ย  The registered nurse is responsible for ensuring that care is delivered appropriately and the standards of quality are adhered to. ย 
The registered nurse is responsible for working with other medical disciplines (i.e., PT, OT, ST) to ensure coordinated services and that all of the clientโ€™s needs are identified through the assessment process and addressed through the implementation of the Plan of Care and re-evaluation and supervisory process. ย 
ESSENTIAL DUTIES & RESPONSIBILITIES
1.ย  ย  ย  ย Provides care and procedures as defined within the scope of nursing practice and State regulations.
2.ย  ย  ย  ย If directed by the Director of Patient Care Services/Administrator conducts the initial evaluation visit and regularly revaluates the clientโ€™s nursing needs.
3.ย  ย  ย  ย Initiates the plan of care and makes necessary revisions, keeping the Director of Nursing/Supervising Nurse updated on the revisions.
4.ย  ย  ย  ย Provides those services requiring substantial and specialized nursing skill.ย 
5.ย  ย  ย  ย Initiates appropriate preventive and rehabilitative nursing procedures.
6.ย  ย  ย  ย Prepares clinical and progress notes and summaries of care.
7.ย  ย  ย  ย Informs the Director of Patient Care Services/Administrator, physician and other personnel of changes in the clientโ€™s condition and needs.
8.ย  ย  ย  ย Counsels the client and family in meeting nursing and related needs.
9.ย  ย  ย  ย Assists in the development of organization goals.ย  Administers agency policies and procedures.ย 
10. ย  Assists the Director of Patient Care Services/Administrator with assessing the competency of clinical staff to promote quality, continuity and safety of a clientโ€™s care.
11. ย  Supervises, educates and coaches other nursing personnel, as directed by the Director of Nursing/Supervising Nurse.
12. ย  Provides information to be used for statistics, reports and records for purposes of evaluation and reporting of organization activities.
13. ย  Assures proper maintenance of clinical records in compliance with provincial and federal laws.
14. ย  Assures compliance with all state and federal lawsย 
15. ย  Identifies and participates in agency performance improvement activities.
16. ย  Stays informed about changes in the field of nursing and home health care.ย  Shares information with appropriate organization personnel.
17. ย  Promotes home health referrals in the health care community.
Experience & Qualifications:

ยทย  ย  ย  ย  ย Current license and in good standing with the State Board of Nursing in the State of California.
ยทย  ย  ย  ย  ย Minimum two-three years of clinical experience with at least half of that time working in home care or related health care organization. ย 
ยทย  ย  ย  ย  ย Wound care certification preferred
ยทย  ย  ย  ย  ย IV certification preferred
ยทย  ย  ย  ย  ย Ability to travel on a daily basis including weekends, nights and holidays
