Job Description:
Job Summary
Responsible for providing comprehensive pharmaceutical care by reviewing and optimizing medication orders, monitoring therapeutic drug regimens, and participating in patient care rounds. Ensures safe, effective, and cost-conscious medication therapy through collaboration with physicians, nurses, and interdisciplinary teams. Delivers patient and staff education, precepts pharmacy learners, and supports clinical research and quality improvement initiatives. Coordinates medication reconciliation, clinical interventions, and pharmacokinetic monitoring to enhance patient outcomes. Maintains adherence to regulatory standards and upholds organizational values in all pharmacy practices.
Essential Functions
• Participates in medication reconciliation to ensure accurate home medication lists.
• Reviews and verifies medication orders for appropriateness, safety, and accuracy.
• Collaborates with providers and multidisciplinary teams for comprehensive medication management and access.
• Educates patients, caregivers, and providers on medication therapy.
• Compounds, dispenses, and supervises the preparation of medications, ensuring quality and compliance.
• Maintains inventory and records for controlled substances and medications.
• Participates in quality improvement, patient safety, and cost management initiatives.
• Provides staff development, preceptorship, and professional enrichment for pharmacy personnel.
• Performs other duties as assigned.
Education Requirement
Bachelor's Degree or PharmD Degree from an ACPE accredited school of Pharmacy
Experience Requirement
0-2 years of experience in pharmacy operations, or a relevant area with knowledge of TJC, USP 797, 795 and 800, and federal and state regulations
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
Active Licensed Pharmacist in applicable states, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) within 90 days of hire
