Job description

We are actively seeking a Clinical Post Acute Care (PAC) Subject Matter Expert (SME) to join our growing healthcare team. If you have deep expertise in post-acute care workflows and clinical processes, this is a great opportunity! Experience Required: 10+ Years (preferred) Location: Remote Role Type: Contract Key Responsibilities Provide SME expertise in Post-Acute Care (PAC) workflows and operations Collaborate with clinical, technical, and business teams Support implementation and optimization of PAC solutions Ensure compliance with healthcare regulations and standards Analyze clinical data and improve patient care processes Work with EHR/EMR systems and healthcare data integration Important Skills (Must Have) Strong knowledge of Post-Acute Care (Skilled Nursing, Home Health, Rehab, LTAC) Experience with clinical workflows & care coordination Hands‐on experience with EHR/EMR systems (Epic, Cerner, etc.) Understanding of CMS guidelines, HIPAA compliance Knowledge of HL7, FHIR, healthcare data standards Strong analytical and problem‐solving skills Excellent communication with clinical & non‐clinical stakeholders Experience in healthcare IT or digital transformation projects Preferred Qualifications Clinical background (RN, PT, OT, or related) Experience in PAC consulting or healthcare systems implementation Certifications in healthcare/clinical informatics are a plus This is a remote position.
