We are seeking a skilled and compassionate Part-Time Physical Therapist (PT) to provide individualized therapy services to our pediatric and adult population. The Physical Therapist is responsible for evaluating, developing, and implementing individualized therapy programs designed to improve mobility, strength, balance, coordination, endurance, positioning, and functional independence for pediatric and adult residents.

The PT collaborates with physicians, nursing staff, respiratory therapists, occupational therapists, speech-language pathologists, caregivers, families, and other interdisciplinary team members to develop evidence-based treatment plans that support each resident’s individualized goals while maintaining safety and quality of life.

Reports to: Director of Nursing (DON)

Schedule: Part-Time

• Perform comprehensive physical therapy evaluations for pediatric and adult residents.
• Develop individualized treatment plans based on physician referrals, resident needs, and established goals.
• Provide therapeutic exercise, gait training, transfer training, balance activities, strengthening, endurance training, and mobility interventions.
• Assess functional limitations and recommend appropriate therapeutic interventions.
• Monitor resident progress and modify treatment plans based on clinical response and functional outcomes.

• Provide developmentally appropriate physical therapy interventions for pediatric residents.
• Assess and support gross motor development, mobility, balance, strength, coordination, positioning, and functional movement.
• Adapt treatment approaches to the developmental and medical needs of pediatric and adult residents.
• Promote each resident’s highest achievable level of independence with mobility and activities of daily living.
• Educate families and caregivers on therapeutic exercises, mobility techniques, positioning, and strategies to support continuity between therapy sessions.

• Provide gait, transfer, positioning, and functional mobility training.
• Evaluate fall risk and implement appropriate prevention strategies.
• Provide wheelchair positioning and seating recommendations.
• Recommend adaptive equipment and mobility devices when clinically appropriate.
• Promote safe participation in everyday activities while supporting resident independence.

• Collaborate with Respiratory Therapists and nursing staff when providing therapy to residents requiring ventilators, tracheostomies, oxygen therapy, or G-tubes.
• Modify therapy interventions based on respiratory status, endurance, medical complexity, and resident safety.
• Utilize positioning techniques that support respiratory function, comfort, mobility, and pressure injury prevention.
• Monitor tolerance to physical activity and communicate changes or concerns promptly to the interdisciplinary team.

• Collaborate with physicians, nurses, respiratory therapists, occupational therapists, speech-language pathologists, case managers, caregivers, and families.
• Participate in interdisciplinary care planning and treatment discussions.
• Communicate resident progress, barriers, and recommendations to appropriate members of the care team.
• Educate caregivers and direct care staff on safe mobility, transfers, positioning, and therapeutic exercises.

• Complete evaluations, treatment notes, progress reports, and discharge documentation accurately and timely.
• Ensure services provided are consistent with authorized plans of care and applicable payer requirements.
• Maintain documentation supporting medical necessity and services provided.
• Maintain HIPAA compliance and resident confidentiality.
• Complete required organizational and regulatory training.
• Participate in quality improvement initiatives and clinical audits.
• Follow applicable reporting requirements for incidents and suspected abuse, neglect, or exploitation.

• Current Physical Therapist (PT) license in the State of Florida in accordance with Chapter 486, F.S.
• Degree in Physical Therapy from a CAPTE-accredited program as required for Florida licensure.
• Current CPR/BLS certification.
• Ability to successfully complete required Level II background screening.
• Eligible to participate in Medicare, Medicaid, and other applicable federal healthcare programs.
• Experience in home health, rehabilitation, long-term care, pediatric care, or medically complex populations preferred.
• Pediatric therapy experience strongly preferred.
• Experience working with individuals requiring ventilator, tracheostomy, oxygen, and/or G-tube support preferred.
• Experience working with individuals with developmental or intellectual disabilities preferred.
• Strong clinical assessment and rehabilitation planning abilities.
• Excellent communication, documentation, and interdisciplinary collaboration skills.
• Compassionate, patient-centered approach to care.
• Ability to adapt therapeutic interventions for both pediatric and adult residents.
• Ability to travel to assigned client homes or group home locations.
• Ability to lift or move 50 pounds or more, with or without reasonable accommodation.
