Job description

The RN Clinic role is a licensed registered nurse who delivers direct patient care, care coordination, and health education within an ambulatory community health clinic setting. This role collaborates with providers and the interdisciplinary team to assess, plan, implement, and evaluate care for patients across the lifespan. The RN Clinic role is responsible for performing clinical procedures, managing acute and chronic conditions, and supporting population health initiatives. The position emphasizes patient advocacy, evidence-based practice, and a commitment to quality and safety in the outpatient environment.
Core Job Responsibilities:
  • Perform comprehensive nursing assessments, obtain patient histories, monitor vital signs, and provide direct patient care within scope of practice and current competencies, including in-person and telephone triage.
  • Administer medications, immunizations, and treatments as prescribed; provide wound care, assist with minor procedures, and manage urgent walk-in triage.
  • Conduct nurse-led visits for chronic disease management (e.g., diabetes, hypertension), women's health, anticoagulation, preventive care (e.g., annual wellness visits), and perform medication reconciliation for moderate/high-risk patients.
  • Utilize standing orders and nursing process to support scheduled nurse-led visits, including diabetes and chronic disease education, pregnancy and women's health, anticoagulation management, and wound care.
  • Participate in and/or lead team huddles to support care planning and communication; serve as a clinical support resource for other care team members.
  • Provide individualized and group patient and family education on chronic disease management, acute conditions, preventive health behaviors, medication management, lifestyle changes, and health maintenance; use motivational interviewing and coaching to support patient goal-setting and self-management.
  • Perform patient outreach and follow-up as directed; coordinate care for complex and high-risk patients using evidence-based protocols; collaborate with primary care providers, specialists, and community partners to ensure continuity and quality of care.
  • Provide coverage at other clinic sites as needed, delivering care within HRSA service scope and approved locations, including patient homes and community-based sites as appropriate, adhering to defined services and safety protocols; local travel required.
  • Maintain accurate, timely, and confidential electronic health records; document all interactions in the EHR thoroughly and promptly; communicate abnormal lab results and treatment plans to patients as outlined by provider protocols; serve as a liaison between patients, providers, and the care team to ensure seamless information flow.
  • Adhere to infection control, safety, and regulatory standards; participate in quality improvement initiatives and staff training.
  • Demonstrate proficiency and act as a role model in patient care and clinic operations; support operational efficiency by prioritizing clinical tasks and assisting with training and competency verification of support staff.

Work Experience: Minimum two years of RN clinical experience in acute or ambulatory care preferred. Care coordination experience in an ambulatory setting preferred. Experience in nurse-led patient visits preferred (i.e. diabetes and chronic disease education, anticoagulation management)
Pediatric RNCC: Experience working with pediatric patients preferred
Education, certification and licensure:
  • Required: Associates degree; Current, unrestricted Oregon RN License; BLS/CPR Certification; Valid Oregon State driver's license
  • Preferred: Bachelor of Science in Nursing (BSN)

Skills & Knowledge:
  • Required: Superior nursing process skills. Critical thinking and problem-solving skills. Excellent written, verbal, telephone and interpersonal communication skills. Familiarity/experience with client interaction on the telephone. Knowledge of community resources. Basic typing and computer skills and comfort with Microsoft Windows operating system.
  • Preferred: EHR experience - EPIC experience a plus. Fluency in Spanish preferred. Involvement with quality improvement processes. Knowledge of health insurance plans, standard office policies and procedures as well as regulatory requirements including CLIA and OSHA standards.
