Role Overview
We are seeking a Primary Care Physician (MD/DO) who owns an existing clinic to provide in-facility care to residents in assisted living communities across the Inland Empire, CA. This role involves mobile in-facility visits with clustered routes to minimize travel time. This is a part-time position starting at a minimum of 1 day per week (Monday through Thursday), with potential to expand based on patient panel growth. This position is available as W-2 or 1099 - we are happy to accommodate what works best for you.
Responsibilities
  • Provide comprehensive primary care to seniors in assisted living and RCFE communities in the Inland Empire area
  • Conduct in-facility patient visits and telehealth consultations as needed
  • Manage chronic conditions, preventive care, and acute concerns
  • Collaborate with our care team including NPs, specialists, and facility staff
  • Maintain accurate and timely documentation in compliance with regulatory requirements

Requirements
  • Must own an existing clinic contracted with IEHP (Inland Empire Health Plan)
  • Active California medical license (MD or DO) in good standing
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Active DEA registration
  • Medicare credentialing preferred
  • Experience in geriatric or senior care settings preferred
  • Valid driver's license and reliable vehicle

Compensation
This is a part-time position starting at 1 day per week with potential to expand based on patient panel growth. Compensation is $800 per day, plus $50 per patient seen.
