Full-Time or Part-Time | $5,000 Sign-On Bonus
Central Coast Home Health is a locally owned, rapidly growing home health organization proudly serving patients throughout San Luis Obispo County and Santa Barbara County. We are seeking compassionate and motivated Registered Nurses to join our team in multiple territories across San Luis Obispo County.
San Luis Obispo/5 cities- Nipomo/Santa Maria/ Coast (Los Osos-Cambria)
As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patients' homes while collaborating with an interdisciplinary care team. This role offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families while managing your own schedule and caseload.
Position Details
  • Full-time or part-time opportunities available (minimum 3 days/week)
  • Flexible scheduling with choice of 4- or 5-day workweek
  • Standard schedule: Monday-Friday, 8:00 AM-5:00 PM
  • Rotating weekend coverage required

Responsibilities
  • Deliver skilled nursing care in patients' homes
  • Assess patient needs and develop individualized care plans

Compensation
This position is compensated on a per-visit basis.
  • Routine Visits: $54.00-$61.00 per visit
  • Evaluations: $92.00-$104.00 per visit
  • Admissions: $162.00-$183.00 per visit

Actual compensation may vary based on experience, territory, visit type, and location.
