Job Summary
Home Services Unlimited is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide in-home care throughout the Indianapolis area. This position offers autonomy, flexibility, and the opportunity to build lasting relationships with patients while managing your own caseload.

Responsibilities
• Perform comprehensive in-home patient assessments (including OASIS)
• Develop, implement, and evaluate individualized care plans
• Provide skilled nursing care and treatments
• Educate patients and families on care plans and disease management
• Coordinate care with physicians and interdisciplinary team members
• Supervise LPNs and ancillary staƯ as needed
• Complete timely and accurate electronic documentation
• Ensure compliance with all regulatory and agency standards
• Participate in team meetings and quality improvement (QAPI)
Qualifications
• Active Indiana RN license
• ASN or BSN from an accredited nursing program
• Home health experience preferred, including OASIS
• Experience with electronic health records (EHR) preferred
