What You'll Do

As a Home Health Registered Nurse, you will play a vital role in improving patient outcomes through skilled nursing care, education, and care coordination. Responsibilities include:

  • Conduct comprehensive patient assessments and ongoing evaluations of symptoms, progress, and care needs.
  • Perform initial home health visits and complete reassessments as needed.
  • Develop, implement, and monitor individualized care plans.
  • Accurately complete and submit OASIS documentation in a timely manner.
