Job Responsibilities:
  • Conduct in-home client assessments and develop individualized plans of care

  • Serve as a clinical liaison between caregivers, physicians, families, and the office team

  • Train, guide, and support caregivers on care plans and safe practices

  • Monitor client progress and revise care plans as needed

  • Ensure compliance with state home care regulations and agency policies


Qualifications:
  • Current RN license in the state of Georgia (Required)
