Job description

You'll provide skilled nursing care in patients' homes, working as part of a full care team, with the kind of one-on-one time that hospital and facility nursing rarely allows.

Schedule

  • Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
  • Full time: two weekends per month, with a weekday off following each weekend worked
  • Part time: one weekend per month

What You'll Do

  • Provide skilled nursing visits to patients in their homes under the direction of the RN case manager

  • Educate patients and families on their care plan, medications, and home safety
  • Document visits thoroughly and in a timely manner
  • Build real relationships with the patients you see week after week

What We're Looking For

  • Active LPN license in Oklahoma
  • Home health experience is a plus, but not required. We welcome LPNs from hospital, facility, or clinic backgrounds who are ready for a change
  • CPR certification
  • Comfortable working independently and managing your own schedule
