JOB TITLE: OBRA Assessor
LOCATION: Petoskey, MI 49770
REPORTS TO: OBRA Supervisor
STATUS: Part-time, Call-in; Non-Exempt, Non-Union
HYBRID ELIGIBLE: Community-Based position
STARTING WAGE RANGE: $40.00 per hour
QUALIFICATIONS:
Education: Masterโ€™s degree in an applied area of social work, psychology, or counseling. Must meet the requirements for Licensed or Limited Licensed Master Social Worker, Licensed or Limited Licensed Professional Counselor, or Licensed or Limited Licensed Psychologist.
Experience: Possess experience in clinical social work with the mentally ill, with expertise in mental health and aging, developmentally disabled individuals, persons with dementia, or other relevant human services work.
Other: Lived experiences with behavioral health issues are valued.
SUMMARY OF RESPONSIBILITIES:
The OBRA Assessor participates as a member of the multidisciplinary team, providing comprehensive Pre-Admission Screenings (PAS) and Annual Resident Reviews (ARR) to persons identified via the OBRA screening process. The OBRA Assessor is responsible for completion of the Psychosocial and Psychiatric part of the OBRA Assessment. If the Assessor is a psychologist, he or she would also be responsible for the Psychological part of the OBRA Assessment.
ESSENTIAL JOB FUNCTIONS:
  1. Assesses individuals referred for Level II OBRA mental health screenings.
  2. Completes reports, paperwork, and communications needed to finalize OBRA mental health assessments.
  3. Provides appropriate clinical documentation for all services provided.
  4. Documents activities in the paper or electronic clinical record and the agency information management system in accordance with agency policies and Medicaid requirements.
  5. Perform other related duties as assigned by supervisor.
  6. Complete the Psychosocial and Psychiatric Assessment part of the Level II Assessment as assigned by the OBRA Supervisor. If the Assessor is a psychologist, they will also complete the Psychological section part of the Level II Assessment.
  7. Directly interview, observe and evaluate the strengths and weaknesses of the consumer.
  8. Provide recommendations to the Agency and/or nursing home staff to carry out programs designed to enhance an individualโ€™s lifestyle.
  9. Review all pertinent written records to gather information for the assessment.
  10. Consult with Agency and nursing home staff, consumer, consumer family members, and other individuals and agencies as indicated to obtain all necessary information to complete the assessment.
  11. Recommend treatment specific to the needs of the individual.
  12. Provide documentation of all billable services completed in the OBRA Level II Assessment process.
  13. Learn new information regarding treatment of individuals with serious mental illness and intellectually and/or developmentally disabled consumers through self-training, reading, in-service, or conferences.
  14. Provide complete and timely documentation in accordance with Agency policies, procedures and OBRA requirements.
  15. Maintains confidentiality of service recipients in accordance with HIPAA Privacy Laws and 42 C.F.R., Part 2.
PHYSICAL REQUIREMENTS:
Ability to walk, bend, stand, sit, lift to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity. Position requires physical ability to complete and maintain certification in de-escalation techniques which will be a training class provided to the job incumbent. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions in accordance with applicable laws.
OTHER REQUIREMENTS:
  • Must be current in all applicable trainings; provided by NCCMH.
  • Have own phone for communication, reliable access to messages and multi-factoring capabilities.
  • Have reliable transportation and be able to drive a company vehicle. Must also possess a valid Michigan driver's license and have a satisfactory driving record.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
  • Supports a philosophy of service delivery that is recovery-based, person centered, and culturally competent.
  • Adhere to applicable policies and procedures as required by the Department of Community Mental Health, the Mental Health Services Board, and other pertinent regulatory agencies.
  • Proficient with Microsoft Windows and the Office Suite, especially Outlook, Word, and Excel; able to maneuver on the Internet to conduct research or obtain information, including EHR and Video Conferencing.
