JOB TITLE: Registered Nurse/OBRA
LOCATION: Petoskey, MI
REPORTS TO: OBRA Supervisor
STATUS: Part-Time, On-Call, Non-Union
HYBRID ELIGIBLE: Yes
STARTING WAGE RANGE: $30.57 - $32.09 per hour
QUALIFICATIONS:
Education: Graduate from an accredited school of nursing and licensed as an RN in the state of Michigan.
Experience: Minimum of 2 years’ experience in nursing, plus 1-year experience working with mental health and/or clients with developmental disabilities or equivalent clinical experience preferred.
Other: The individual will demonstrate a basic understanding of the aging process and factors which may impact on the mental health of the consumer. Lived experiences with behavioral health and/or developmental disability issues are valued. Ability to maintain the competency level as defined within the job classification and specific clinical practice. Ability to communicate professionally, verbally and in writing.
SUMMARY OF RESPONSIBILITIES:
Under the direction of the OBRA Coordinator, the OBRA Registered Nurse is responsible for timely and thorough completion of the Summary of Medical History and Examination section of the OBRA Preadmission Screening and Annual Resident Review (PASARR) process which facilitates fulfillment of federal OBRA mandates for nursing facilities. The assessment will identify the physical, psychiatric, and mental health status to determine appropriate level of care required in a community or facility setting. The assessment covers the following areas: General Information, Current Medical Axis III Diagnoses, Current Medications, Medication History, Special Treatments and Procedures, Physical Assessment, Abnormal Lab Values During the Last Six Months, Surgeries/Diagnostic Procedures During the Previous Year, Functional Assessment, and Recommendations. The OBRA Registered Nurse’s recommendations are incorporated into the nursing home’s Care Plan as well as the individual’s agency Plan of Service if appropriate.
ESSENTIAL JOB FUNCTIONS:
  • Completes the Summary of Medical History and Examination section of the OBRA Assessment as assigned by the OBRA Coordinator.
  • Directly interviews, observes, and evaluates the functional strengths and weaknesses of the consumer.
  • Provides recommendations to agency and/or nursing home staff to carry out medical and physical strategies designed to enhance an individual’s lifestyle.
  • Reviews all pertinent records to gather information for the OBRA Assessment.
  • Provides documentation of all billable services completed in the OBRA Assessment process.
  • Consults with agency and nursing home staff, consumer, consumer family members, and other individuals and agencies as indicated to obtain all necessary information to complete the OBRA Assessment.
  • Learns new information regarding treatment of mentally ill, intellectually and developmentally disabled clients through self-training, reading, in-service or conferences.
  • Provides complete and timely documentation in accordance with agency policy and procedure and OBRA requirements.
  • Performs other related duties as assigned by OBRA Supervisor and/or Director of Health Services.
  • Maintains confidentiality of service recipients in accordance with HIPAA Privacy Laws and 42 C.F.R., Part 2.
PHYSICAL REQUIREMENTS:
Ability to walk, bend, stand, sit, lift up to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in CPI techniques which will be a training class provided to the job incumbent.   • Must be able to complete and remain current in initial and ongoing training requirements, including Adult CPR and First Aid.
  • Have own phone for communication, reliable access to messages, and multi-factoring capabilities.
  • Have reliable transportation in carrying out agency duties.
  • Possess a valid Michigan driver’s license and have a satisfactory driving record.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
  • Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent.
  • Adhere to applicable policies and procedures as required by MDHHS and other pertinent regulatory agencies.
