Position Summary We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team. In this role, you will provide high-quality nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary team to ensure the best possible outcomes for our residents. Key Responsibilities
  • Provide direct nursing care in accordance with physician orders and individualized care plans
  • Administer medications and treatments safely and accurately
  • Monitor residents and promptly report changes in condition
  • Maintain accurate and timely clinical documentation
  • Supervise and support Certified Nursing Assistants (CNAs)
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members
  • Respond appropriately to resident needs, concerns, and emergencies
  • Promote resident dignity, comfort, and quality of life
  • Ensure compliance with all federal, state, and facility regulations
  • Utilize PointClickCare (PCC) and other clinical systems as needed
Qualifications
  • Current, unrestricted LPN license in South Carolina
  • Current CPR certification
  • Must maintain all required continuing education and licensure requirements
  • Must remain in good standing with the South Carolina Board of Nursing
  • Experience in skilled nursing or long-term care preferred
  • Experience with PointClickCare (PCC) preferred
  • Strong clinical, communication, and organizational skills
