Job description

Harmonious Care Services is a home healthcare agency dedicated to providing exceptional, personalized home healthcare that empowers our clients to live with dignity, independence, and improved quality of life. We are looking for a Skilled Registered Nurse (RN) who will play a key role in offering thoughtful, one-on-one care to our clients in their homes. S/he will support each clientโ€™s unique goals and ensure each client is well cared for feel truly seen, and heard.

This role is on a PRN basis and the RN would work with us within the Springfield, IL area. Based on your preferred location, we would work to build a caseload for you in these areas.

Duties and Responsibilities:

- Visit clients in their homes to complete initial assessment, create a care plan to determine home care needs, and provide direct patient care

- Develop care plans which establish goals based on nursing needs and incorporate therapeutic, preventive, and rehabilitative nursing actions. The RN will include the client and their family in planning process and counsels them on how to meet the goals set forth.

- Regularly re-evaluate the clientโ€™s nursing needs and supervise LPNs and HHAs working with you to care for the client

- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the client

- Maintain appropriate clinical records and notes, and report to the physician any changes in the clientโ€™s condition. Submit these reports in accordance with our companyโ€™s policies and procedures and OASIS documentation procedures.

- Collaborate closely with a multidisciplinary team of physicians, and other wellness professionals to ensure a seamless and supportive care experiences for every client.

- Undertake skilled nursing services as it relates to home healthcare services including: disease management, medication management/teaching/administration, postoperative management, pain, management, infusion therapy, lab draws, tracheostomy care, foley catheter care, nasogastric, tube care, intravenous line insertion and care, wound care and wound V.A.C. care, measurement of vital signs (blood pressure, pulse, respiration rate, lungs sounds, pulse oximetry, and glucose) as ordered by physician.

Qualifications:

- A registered nurse with a valid license to practice in the State of Illinois

- At least one (1) year of appropriate experience as an RN in a home healthcare setting

- Demonstrate good verbal and written communication and organizational skills

- Possess and maintains a current CPR certification

- Can provide a copy of a negative TB test

- Must be a licensed driver with an automobile that is insured in accordance with state and organizational requirements and is in a good working condition.

- Knowledge and experience using EMR and documentation software tools.
