Deliver Comprehensive Primary Care to Underserved Seniors
A growing value-based healthcare organization is seeking a Part-Time Nurse Practitioner to provide comprehensive primary care for Medicare-Medicaid (dual-eligible) patients. This role offers the opportunity to care for medically complex older adults while practicing in a collaborative, patient-centered environment that prioritizes quality over volume.
You'll have the time and support to build meaningful relationships with your patients, educate families, manage chronic conditions, and improve health outcomes through preventive, coordinated care.

What You'll Do
You'll provide comprehensive primary care to an assigned panel of patients while partnering with an interdisciplinary care team to improve outcomes and reduce unnecessary healthcare utilization.
Responsibilities include:
  • Manage chronic medical conditions including diabetes, hypertension, chronic kidney disease (CKD), heart disease, and other complex illnesses
  • Perform comprehensive health assessments and annual wellness evaluations
  • Conduct follow-up visits to assess symptoms, review laboratory results and vital signs, and adjust treatment plans
  • Develop individualized care plans based on each patient's medical, behavioral, and social needs
  • Educate patients and caregivers on disease prevention, medications, nutrition, and lifestyle modifications
  • Support medically complex patients in navigating the healthcare system
  • Coordinate care with physicians, specialists, caregivers, and community resources
  • Order and interpret diagnostic testing as appropriate
  • Accurately document patient encounters, including HCC coding and value-based care documentation
  • Practice independently while collaborating closely with physicians and an integrated care team
  • Maintain compliance with HIPAA and all regulatory requirements

This full-time position includes:
  • Primary care clinic visits
  • In-home patient visits within approximately a 30-mile service area
  • Telehealth appointments as appropriate

Occasional travel to nearby service areas may be required to support patient care and scheduling needs.
Requirements
What We're Looking For
Required
  • Active California Nurse Practitioner license
  • Board Certification through AANP or ANCC
  • Current DEA certification or ability to obtain
  • Current BLS/CPR certification
  • Minimum two years of Nurse Practitioner experience in one or more of the following:
    • Primary Care
    • Family Medicine
    • Internal Medicine
    • Geriatrics
    • Palliative Care
  • Experience managing patients with multiple chronic conditions
  • Working knowledge of HCC coding and value-based care documentation
  • Bilingual in English and Spanish
  • Strong patient education and communication skills
Preferred
  • Experience caring for Medicare-Medicaid (dual-eligible) populations
  • Home-based primary care experience
  • Experience working with underserved or vulnerable populations
  • Care coordination or population health experience
