Overview

Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Little Falls, MN consists of 2 cottages/suites and up to 26 residents. We provide the utmost quality of care with an emphasis on person-centered care planning in our home like atmosphere. Are you ready to lead, inspire, and make a difference? We're looking for passionate individuals to step into leadership roles and help shape the future of our organization.

The Licensed Practical Nurse (LPN) is responsible for:
Medication management & chart auditing
Performing resident treatments as ordered per the care plan
Observing, tending to and documenting residents health needs
Communicating with RN Director regarding relevant staff and resident information and/or concerns
Reviewing appointments & setting up transportation for residents
Processing MD orders completely and accurately
Completing incident reports as needed
Ensure the staff is performing safe practices
Executing tours for potential admits
Reviewing messages and communicating with staff through R-Task

Qualifications:
Graduate from approved LPN program
Possess a valid Minnesota LPN License
Minimum of 1 year experience

Requirements:
Leadership ability
Must be able to occasionally fill in aide shifts when needed, this could include weekends.
Maintaining confidentiality and professionalism at all times
Must be able to work as a team member and maintain a positive demeanor
Positive work attitude
