Rate: $34/ hour

Chautauqua Nursing & Rehabilitation Center is looking to add a Graduate Nurse (GN) to its nursing team. In this role, you'll provide nursing care and support to residents under the direction of a Registered Nurse while gaining valuable clinical experience in a skilled nursing environment. RESPONSIBILITIES:

  • Provide nursing care and services to residents in accordance with individualized care plans
  • Administer medications, treatments, and clinical procedures as directed and within scope of practice
  • Monitor resident conditions and accurately document care, observations, and outcomes
  • Communicate changes in resident condition to physicians, nursing leadership, and designated family members as appropriate
  • Receive and provide shift reports and conduct resident rounds to ensure quality care
  • Collaborate with interdisciplinary team members to support resident health and well-being
  • Maintain accurate medical records and ensure documentation reflects resident status and care provided
  • Follow facility policies, infection control practices, HIPAA requirements, and safety procedures
  • Perform additional duties as assigned under the supervision of a Registered Nurse

REQUIREMENTS:

  • Minimum 18 years of age
  • High School Diploma or equivalent preferred
  • Graduate Nurse currently eligible for licensure in New York State
  • Strong written and verbal communication skills
  • Ability to follow written and verbal instructions
  • Ability to work effectively with residents, families, physicians, and team members
  • Compassionate, resident-centered approach to care
  • Physical ability to perform frequent activity and lift a minimum of 20 pounds
