WHAT YOU'LL DO AS OUR PATIENT CARE COORDINATOR:
As the weekend Patient Care Coordinator, you begin each shift by aligning staffing, solving call-outs, and ensuring the day flows smoothly. You step directly onto the floor to model best practices, help with onboarding and logins, provide hands-on training, and coach staff in communication, redirection, and dementia-care techniques. Families rely on you when the Manager is away, trusting you with care conferences, meetings, tours, and compassionate problem resolution. You collaborate with life enrichment to keep programming meaningful and encourage team participation in engagement and routines. Your eye for detail keeps operations sharp: completing documentation, reports, chart audits, filing, managing the list of expired standing orders, supporting admissions, transfers, and discharges, tracking supplies (first aid, gloves, bags), and even putting spools away every other Friday. Throughout it all, you respond to call lights quickly, support new move-ins, fill in for call-offs when needed, provide steady weekend oversight, and step in to manage the unit confidently when required.
