About Sunnyvale Gardens Post Acute

Sunnyvale Gardens Post Acute is a premier 140-bed Medicare-certified skilled nursing and rehabilitation facility. Now operated by PACS, our facility is committed to providing industry-leading clinical care to our residents. We are recognized for our innovative clinical programs, including a specialized respiratory therapist-led program and being a unique provider of peritoneal dialysis in the Northern California region.

Our mission is to maximize the quality of life for our long-term residents and provide high-intensity rehabilitation to help short-term patients return home successfully. We are looking for compassionate, skilled RNs to join our interdisciplinary team during the PM and NOC shifts.

The Role

As a Part-Time Registered Nurse at Sunnyvale Gardens, you will provide high-quality direct nursing care to a diverse patient population. You will play a vital role in managing the transition from acute hospital care to home recovery, ensuring clinical oversight is maintained around the clock.

Key Responsibilities:

  • Administer medications, provide wound care, and perform treatments in accordance with physician orders and customized care plans.Direct Clinical Care:

  • Observe residents' health status, monitor vital signs, and promptly document changes in the medical record.Monitoring & Assessment:

  • Supervise Licensed Vocational Nurses (LVNs) and Certified Nursing Assistants (CNAs) on shift to ensure the highest standards of care.Team Leadership:

  • Coordinate with on-site physical, occupational, and speech therapists, as well as consulting pharmacists and dietitians, to align treatment goals.Collaboration:

  • Provide care for residents within our specialized respiratory and dialysis programs.Specialized Support:

    Requirements

    • Licensure: Active and unrestricted Registered Nurse (RN) license issued by the California Board of Registered Nursing.

    • Certification: Current BLS/CPR certification.

    • Experience: Previous experience in a skilled nursing, post-acute, or long-term care setting is preferred.
