GENERAL OVERVIEW:

This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides specialty and critical care healthcare services in physician office(s) and non- acute based settings with a focus on chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Specialty certification, advanced procedures with appropriate privileging, weekend and on-call coverage on a rotational basis, as required. E-consults and telehealth could be included in the care provided.


ESSENTIAL RESPONSIBILITIES

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation for patients in physician office(s) and non- acute based settings.(30%)

  • Communicates with attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. Makes medical diagnoses and institute appropriate therapy within the applicable scope and standards of practice.(15%)

  • Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies appropriate for the patient's condition to determine and implement treatment plans.(10%)

  • Assesses learning needs of patient and family and provides education based on age, culture and willingness to learn. Educates patients and family members regarding health and illness prevention, medication and treatment instructions, and recommend community resources as needed.(15%)

  • Ensures appropriate understanding and timely documentation in the EMR to support accurate medical coding and billing, and record/report pertinent information to the attending physician of record.(10%)

  • Maintains mandatory professional continuing education, actively participate in patient safety, quality improvement, and patient engagement initiatives, and maintain professional practice consistent with organizational and advanced practice mission statements and specialty-specific privileges.(10%)

  • Perform other duties as assigned or required.(10%)

QUALIFICATIONS:

Required

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program

  • National Board Certification for Nurse Practitioners through an accredited association

  • Practicing-state licensure as a Nurse Practitioner

  • Prescriptive Authority from the practicing state Within 60 days of hire
