Licensed Practical Nurse/ PDN Nurse

Qualifications

  • Graduate of an accredited Practical/ Vocational program
  • Minimum of two (2) years of experience as a Professional Nurse in home health care setting
  • Knowledge of State and Federal regulations
  • Proficient oral and written communication skills
  • Considerable knowledge of the care and treatment of patients and special procedures that apply to practical nursing
  • Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
  • Ability to independently seek out resources and work collaboratively
  • Ability to work with a variety of groups; multidisciplinary teams, managers, and staff
  • Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/ or equipment over 50 pounds

Duties and Responsibilities shall include but limited to the following:

  • Adhering to the LPN scope of practice as set forth by the State Board of Nursing
  • Preserving the dignity and self-respect of patients
  • Practicing patient privacy and confidentiality
  • Performing specific treatments as ordered by the MD while under RN supervision
  • Administering prescribed medication within the state scope of practice
  • Collaborating with other health care providers and provide education to patients and/ or significant others
  • Documenting and reporting care received, changes, observations and other data related to the clinical condition of the patient to the RN
  • Performing other duties as requested
