Job description

Field Nurse (LPN/RN) – Home Health

We are seeking dependable and versatile Field Nurses (LPN/RN) to provide quality patient care in the home health setting. Field Nurses are a vital part of our agency and play a key role in ensuring excellent patient outcomes.

Pay:

  • $30.00 – $36.00 per hour

Schedule:

  • Tuesday, Wednesday, Thursday: 3:00 PM – 8:00 PM

  • Saturday: 9:00 AM – 7:00 PM

Qualifications:

  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in Virginia or valid multi-state compact license

  • Home health or home care experience preferred

  • Ability to work independently and manage time effectively

  • Reliable transportation and strong documentation skills

Responsibilities:

  • Provide direct patient care in the home according to physician orders and plan of care

  • Follow, assess, and reevaluate patient care needs

  • Complete all clinical nursing documentation accurately and on time

  • Perform skilled nursing services as required

  • Work assigned shifts and complete patient visits as scheduled
