Community-Based Outreach Position

Who We Are Looking For

The Transgender Health & Wellness Center (TH&WC) is seeking a highly connected, community-rooted Transgender Community Outreach & Patient Navigator to expand access to affirming healthcare, social services, and supportive resources throughout Vista and the greater North County San Diego area.

This is not simply a traditional outreach position. We are looking for someone who knows the local transgender, nonbinary, and gender-diverse community, understands the community's needs, and has established credibility and relationships within the community.

The successful candidate will identify transgender and gender-diverse individuals who may benefit from TH&WC services, build trusting relationships, provide navigation and referrals, conduct community outreach, and represent TH&WC at resource fairs, community events, and other outreach opportunities.

Required Qualifications

Candidates should possess:

• At least three years of demonstrated direct engagement, advocacy, outreach, organizing, peer support, navigation, or service within transgender, nonbinary, and gender-diverse communities.
• A high level of demonstrated cultural competency regarding transgender and gender-diverse communities, including familiarity with terminology, barriers to care, healthcare disparities, social determinants of health, and issues affecting different segments of the community.
• Strong existing connections to the Vista/North County San Diego transgender and LGBTQ+ community and knowledge of local community networks, organizations, resources, events, and gathering spaces.
• Demonstrated commitment to social justice, health equity, advocacy, and the advancement of transgender and gender-diverse communities.
• Experience building authentic relationships with community members, particularly individuals who may be disconnected from traditional healthcare or social-service systems.
• Ability to communicate effectively with individuals from diverse backgrounds while maintaining professional boundaries and confidentiality.
• Ability to independently conduct community outreach and proactively identify new opportunities to reach transgender and gender-diverse individuals.
• Ability to navigate prospective patients and clients from initial contact through successful connection with TH&WC services.

Strongly Preferred

• Recognition as a local transgender-community leader, advocate, organizer, peer leader, or other trusted community resource is strongly preferred but not required.
• Existing relationships with transgender and LGBTQ+ organizations, community groups, healthcare providers, colleges, social-service organizations, support groups, and community leaders throughout North County San Diego.
• Previous experience with patient navigation, community health work, peer navigation, outreach, healthcare, social services, or nonprofit organizations.
• Experience staffing resource tables, conducting presentations, organizing community events, and developing community partnerships.

Core Responsibilities

The Transgender Community Outreach & Patient Navigator will:

• Conduct direct outreach throughout Vista and North County San Diego.
• Develop relationships with transgender and gender-diverse community members.
• Identify individuals who could benefit from TH&WC's healthcare and social-service programs.
• Educate community members about available TH&WC services.
• Navigate prospective patients through the process of accessing services and completing appropriate referrals.
• Conduct follow-up to help ensure individuals successfully connect with services.
• Host and staff TH&WC resource tables at community events, health fairs, Pride events, colleges, community organizations, and other appropriate locations.
• Build and maintain relationships with local LGBTQ+ organizations, healthcare providers, community organizations, advocacy groups, and other referral partners.
• Identify new outreach opportunities rather than waiting for outreach opportunities to be assigned.
• Maintain accurate documentation of outreach contacts, referrals, navigation activities, events, and outcomes.
• Work collaboratively with TH&WC staff to identify gaps in services and emerging community needs.
• Represent TH&WC professionally and respectfully in the community.

What Success Looks Like

This is a results-oriented community outreach position. Success is measured not simply by attending events or distributing materials, but by developing meaningful community relationships and successfully connecting transgender and gender-diverse individuals with services.

The successful candidate will be expected to demonstrate consistent community engagement, develop new referral relationships, maintain an active presence within the local community, conduct resource-table outreach, follow up with prospective patients, and contribute to measurable growth in the number of individuals accessing TH&WC services.
