Compensation: $24-$28 per hour
Location: Golden Valley, Minnesota
Schedule: Part-Time | Friday-Sunday or Friday-Monday | Every Weekend Preferred (Every Other Weekend Considered)
Training: Monday-Friday | 8:00 a.m.-5:00 p.m. for the first several weeks

Home Health Care, Inc. is looking for a Weekend Intake Coordinator who enjoys being at the center of a fast-moving healthcare operation. You'll help take new referrals from "we have a patient who needs care" to "we have a clinician ready to see them."

You'll Own the Weekend Intake Process

This isn't simply answering phones or entering information.

You'll help manage the moving pieces that determine how quickly a new patient can begin receiving care.

Responsibilities include:

Review and process incoming home health referrals while determining whether required information is complete

• Communicate with hospitals, clinics, physicians, referral partners, patients, and families to gather missing information

Verify insurance and payer eligibility and help identify potential reimbursement requirements

• Coordinate nursing, therapy, and other disciplines based on physician orders and patient needs

Schedule Start of Care and other required visits with the appropriate clinical team members

• Accurately enter and maintain patient information within the EMR

• Track outstanding documentation and follow up with referral sources when additional information is needed

• Communicate new admissions and important changes to clinicians and internal team members

• Manage multiple referrals simultaneously while determining what needs immediate attention

• Provide responsive, professional service to referral partners and families throughout the intake process

Required

• Previous healthcare office, coordination, scheduling, admissions, or similar experience

• Strong computer and EMR skills

• Excellent written and verbal communication

• Strong attention to detail and follow-through

• Ability to manage changing priorities in a fast-paced environment

• Availability to complete initial Monday-Friday, 8:00 a.m.-5:00 p.m. training

Highly Preferred

Home health intake or admissions experience

• Home health scheduling or referral coordination experience

• Insurance verification and payer knowledge

• Familiarity with Medicare-certified home health

• Experience working with hospitals, clinics, physician offices, or healthcare referral sources
