Job description

We are seeking a dedicated and compassionate Hospice RN Case Manager to join our team in the North Easton area. As a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families.

Full-time and part-time openings! Weekly pay up to $51 an hour, depending on experience.

Responsibilities:

  • Supervise hospice aides

  • Complete comprehensive assessments of patient and family to determine hospice needs

  • Provide professional nursing care by utilizing all elements of the nursing process

  • Develop and implement individualized care plans

  • Collaborate with interdisciplinary team members to ensure comprehensive care

  • Provide health care instructions to patients and families

  • Counsel patients and families on meeting nursing and related needs

Requirements:

  • Graduate of an accredited school of nursing

  • Must possess an active and unencumbered RN license in Massachusetts
