Position Summary of a Licensed Vocational Nurse (LVN):

  • Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
  • Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
  • Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).

Minimum Qualifications:

  • Current CA Licensed Vocational Nurse (LVN) license
  • Current IV Certification
  • Current CPR card for Healthcare Providers
  • 1 year of Licensed Vocational Nurse (LVN) experience, with Hospice experience preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  • Demonstrates excellent verbal, written communication, and organization skills.
