We are seeking a Part-Time/PRN Physical Therapist (PT) to join our home health team in Valparaiso, Indiana Area

What You'll Do
  • Perform in-home physical therapy evaluations and treatments
  • Develop and implement individualized rehabilitation plans of care
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients and caregivers on exercises, mobility, and safety
  • Document visits accurately and efficiently in accordance with agency guidelines
