JOB SUMMARY:

CNAs and PCAs provide nonmedical service and assistance to a client in their home and community,who, because of advanced age or physical or mental disability, cannot perform these services. Theseservices enable the client to remain in his/her residence. Therefore, the caregiver must feel comfortablecaring for a client in a private home or setting. The Caregivers main objective is client satisfaction andsafety for both oneself and for the client. The Caregiver must have sensitivity for the needs of others, aprofessional manner, and a positive attitude. Aides are responsible for ensuring that service is deliveredin a caring and respectful manner, in accordance with relevant Agency polices, industry standards, andstate regulations. Aides needs to report on time to scheduled shift unless specific circumstances comeup.

ESSENTIAL FUNCTIONS:

The CNA/PCA provides service to a client based on and limited to a client's care plan developed by the RN,the client, and/or the client's family. The care plan is based on an assessment of the client's needs forservices, developed in conjunction with a functional assessment and with the input from the client and/orclient's family. CNA/PCA services are tasks that are necessitated by a client's disability or chronic conditionthat limits the client's ability to perform functional activities or daily living (ADL's) and instrumentalactivities of daily living (IADL's). CNA/PCA services vary depending on the needs and requirements of eachindividual client. Client needs and requirements are documented through the client's functionalassessment. These services involve direct care from cueing/prompting to total assistance.

Aides will provide excellent client care including, but not limited to, assistance with the following:

Ambulation Range of Motion Bathing
Personal Hygiene Dressing Grooming
Feeding Exercise Transferring
Positioning Toileting and Incontinence Care Medication Reminders
Housekeeping Meal Planning and Preparation Laundry
Transportation Shopping and Errands Companionship
Vital Signs and Blood Pressure


CNAs and PCAs will:
Ensure client's safety and security by supervising the home environment
Teach/Perform Meal Planning and Preparation and serving the client
Routine Housekeeping activities such as making/changing bedding, dusting, vacuuming, washingfloors, cleaning kitchen and bathroom, and laundry. Household tasks should not exceed 20% of timeworked per shift unless the client has a Housekeeping Authorization.
Provide Companionship including social interactions, conversations, emotional reassurance, andencouragement of activities that simulate the mind.
Provide respite care for families in accordance with care plans
Preform/assist with essential shopping/errands, which may include handling the client's money inaccordance with the care plan, agency policy, and under the observation of the Supervisor orAdministrator*
Assist clients with following written, special diet plan, and reinforcement of diet maintain ace, whichis provided under the direction of a Physician and as identified on the care plan
Assist clients with standing, walking, sitting, and rolling in bed
Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan*
Assist clients with communication by writing or typing correspondence for them or researchinginformation for them.
Participate on the Care Team by providing input and making suggestions.
Notifies agency when unable to work scheduled assignments - 24 hour notification required.
Ensure service is delivered in accordance with all relevant policies, procedures, and practices.
Monitor supplies and resources.
Follow the written care plan and works all hours as established by Supervisor.
Carry out duties as assigned by the Supervisor.
Observe clients and their environments and immediately reports unsafe conditions to Supervisor.
Observe clients and their environments and immediately reports behavior, physical and/or cognitivechanges and/or changes in living arrangements to Supervisor.
Complete and maintain records of daily activities, observations, and direct hours of service.
Submits written documentation to the office, as required.
Attend orientation, in-service training sessions, and staff meetings.
Develop and maintain constructive and cooperative working relationships with others.
Make decisions and solve problems.
Communicate regularly with Supervisor and co-workers.
Observe, receive, and obtain information from relevant sources.
Report any incidents or accidents immediately to Supervisor.
Consistently follows Code of Conduct and policies as set for in the Employee Handbook.
Maintain client's protected health information confidential and observes all client's rights.
Report, detect, and prevent abuse and neglect.
