Position Summary

We are seeking a dedicated Home Health RN to join our team. This role includes performing skilled nursing visits, patient assessments, wound care, medication management, and care coordination. You will work independently but remain strongly supported by our office and clinical leadership.
Key Responsibilities

  • Perform in-home skilled nursing visits
  • Complete comprehensive assessments and ongoing evaluations
  • Administer medications, wound care, injections, and other skilled procedures
  • Update and follow individualized care plans
  • Communicate with physicians and interdisciplinary team members
  • Provide patient and caregiver education
  • Document accurately and timely using EHR
  • Travel within Santa Ana, Anaheim, and nearby areas
Qualifications

  • Active California RN license (required)
  • Home Health or acute care experience preferred
  • Strong assessment and clinical judgment skills
