Location: Sterling, Illinois
Schedule: Weekdays - PRN Visits Between Monday-Friday 8a-5p with flexibility

Job Summary:
The Home Health Admission RN is responsible for initiating patient care through Start of Care (SOC) admissions, conducting supervisory visits for LPNs in their assigned region every 30 days, and completing recertifications following 30 days of care or post-hospitalization. This role also includes providing skilled nursing visits. The ideal candidate demonstrates strong clinical judgment, organizational skills, and the ability to manage a high volume of documentation with precision and efficiency.
