Offering skilled nursing and therapy services, Bethany Village Home Health Care makes it easier for seniors to recover at home and live independently. Caregivers can feel peace of mind knowing their loved one is being carefully and faithfully served

This will be a Part-Time  day shift position

Pay: $13 and up (additional competitive mileage pay and cell phone stipend)

Position Summary:

Provides Personal Care Services to clients when necessary to maintain their optimum level of functioning assisting with activities of daily living within Graceworks at Home/Health at Home services. Continually works to promote Client independence

Job duties:

  • Provides scheduled personal care needs of the highest quality to clients in their living environment under the Graceworks at Home/ Health at Home Care Program.
  • Under the direction of a registered nurse or therapist, provides care in response to the outlined care plan in the home. This includes; hands on care, assisting with medications reminders that are ordinarily self-administered, assistance in ambulation or exercise, and the performance of simple procedures as an extension of therapy or nursing services, as permitted by State of Ohio practice acts.
  • Attempt to meet the physical, emotional and spiritual needs of clients.
  • Complete paperwork needed to support services and billing requirements.
  • Attend scheduled in-services, training hours and staff meetings.
  • Responsible for promptly reporting any changes in client’s condition (such as skin breakdown, weigh changes, etc), incidents, theft, damage, and/or abuse to the Administrator of Health at Home, LLC
  • Adhere to all policies and procedures of the organization and all federal and state laws.
