Field LPN – Home Health Visits

Location: Orange, Seminole & Osceola Counties, FL | Part-Time / Per Visit

Position Summary

The Field LPN will provide safe, effective, and compassionate nursing care to patients in their homes under the supervision of an RN. You'll follow the individualized Plan of Care, ensuring patients' safety, well-being, and satisfaction. Experience with WellSky/Kinnser and home health is a plus.

Key Responsibilities

  • Deliver care in accordance with the patient's individualized Plan of Care under RN supervision.
  • Provide prompt communication of changes in patient condition/emergencies to the team.
  • Perform skilled nursing tasks within LPN scope of practice, which may include:
    • Medication administration
    • Wound care
    • Infusion therapy
    • Transferring & positioning patients
    • Catheter care
  • Communicate effectively with patients, families, and interdisciplinary team members.
  • Demonstrate empathy, professionalism, and attention to detail while prioritizing patient needs.

Required Experience & Education

  • Current LPN license in the State of Florida (required)
  • IV Certification (required)
  • Strong wound care skills
  • 1+ year of home health experience preferred
  • Excellent communication, teamwork, and collaboration skills
  • Proficiency with electronic health records (EHR)
