Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Registered Nurse to provide skilled nursing care for patients in their homes throughout the Gainesville and Ocala area. The Schedule for the RN PRN is 5-10+ visits per week and some weekend/holiday on-call rotations.

What You'll Do
  • Perform comprehensive patient assessments, including OASIS assessments at required intervals
  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled nursing services and treatments as outlined in the plan of care
  • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
  • Graduate of an approved nursing program with an active Florida RN license
  • Prefers BSN or MSN degree
  • Requires home health RN experience.
  • Prefers experience using Homecare Homebase (HCHB) EMR
  • Strong assessment, critical thinking, and patient education skills
  • Willingness to participate in weekend and holiday on-call rotations
