What You'll do:
As a Home Health RN, you'll deliver skilled, individualized nursing care to patients in the comfort of their own homes- coordinating care, building trust , and making a measurable impact every day.
Key Responsibilities
• Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
• Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
• Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
• Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
• Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
• Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
• Responds promptly to emails and phone calls
