We're looking for a part time Home Health Case Manager who's organized, clinically skilled, and ready to join a supportive team that values autonomy, teamwork, and work-life balance.
What You'll Be Doing
  • Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
  • Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.
  • Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.
  • Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.
  • Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.
