This opportunity is perfect for nurses who are confident working independently, experienced with OASIS documentation, and passionate about helping patients thrive at home.

$48 per Visit | $83 per Open

What you’ll do:

Complete patient assessments and develop individualized care plans

Perform OASIS assessments and maintain accurate documentation

Provide skilled nursing care, treatments, and medication administration

Educate patients and families on care and disease management

Coordinate care with physicians and interdisciplinary teams

Support discharge planning and ongoing patient needs
