Remember when doctors made house calls? At Visiting Physician Services (VPS), that tradition never ended - it evolved. For more than 20 years, we've provided highโ€‘quality, homeโ€‘based primary care to older adults who are homebound, medically complex, or living with chronic conditions. As part of the Parker Advanced Care Institute at VNA Health Group, our interdisciplinary team of physicians, nurse practitioners, and physician assistants delivers comprehensive inโ€‘home medical care, including routine visits, chronic disease management, telehealth, diagnostics, and wound care. Our goal is simple: to ensure patients remain safe, comfortable, and confidently supported in the place they call home.
Position Summary:
Responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families
Principal Responsibilities:
  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses, diagnoses and develops appropriate plan of care to treat homebound patients
  • Monitors the patient's condition and responsiveness to the treatment provided
  • Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations
  • Participates in organizational wide QAPI program and attends agency sponsored in-service programs
  • Adheres to agency infection control and safety policies
  • Adheres to the organization's policy in regards to absenteeism and appearance

Job Competencies & Minimum Qualifications:
  • Master's degree in Nursing required from an NLN accredited program
  • Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
  • National certification as an Adult or Geriatric Nurse Practitioner
  • 2 + years of APN experience and/or significant experience as a registered nurse
  • Excellent communication, organizational and interpersonal skills
  • Strong computer skills

Working Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver's license and automobile insurance

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $120,000 to USD $170,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
