Job description:

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to residents in assisted living communities. This role focuses on preventive care, chronic disease management, and improving the overall quality of life for geriatric patients in a collaborative, patient-centered environment. Enjoy a predictable weekday schedule with no nights or weekends while building meaningful relationships with residents and their families.

Key responsibilities:

  • Perform comprehensive health assessments, physical examinations, and medication reviews.

  • Diagnose and manage acute and chronic medical conditions.

  • Develop individualized treatment and care plans in collaboration with the interdisciplinary care team.

  • Order, interpret, and review laboratory tests and diagnostic studies.

  • Prescribe medications and treatment plans within the scope of New York State NP practice.

Qualifications:

  • Master's Degree in Nursing from an accredited Nurse Practitioner program.

  • Active New York State Nurse Practitioner license.

  • Active DEA registration.

  • Currently enrolled and credentialed with both Medicare and Medicaid.

  • Ability to practice independently in New York State (minimum 3,600 clinical practice hours for Nurse Practitioners).

  • Minimum 3 years of primary care clinical experience.

  • Strong clinical assessment, communication, and documentation skills.

Schedule:

  • Full-Time: 30 hours per week, 4 days per week.

  • Monday-Friday.

  • No Nights. No Weekends.

  • On-site position.
