Job description

About the Role:
Join our dedicated team at JENNIFER GWOZDZ MD PA as a Family Nurse Practitioner in Katy, TX. This role offers an exciting opportunity to provide compassionate care and make a meaningful impact in the lives of our patients.
This is a part time evening position in a Walk in Family practice in Katy - 4-6 hours weekly- 5pm to 7pm , 1-2 evenings a week and 1 Saturday morning a month 9am to 12 noon.

Responsibilities:
  • Conduct comprehensive health assessments and develop patient care plans.
  • Diagnose and treat acute and chronic illnesses in patients of all ages.
  • Perform routine physical exams and preventive health screenings.
  • Prescribe medications and manage treatment plans effectively.
  • Educate patients and families on health management and disease prevention.
  • Collaborate with physician.
  • Maintain accurate and up-to-date patient records in electronic health systems. We use Athena.
  • Participate in quality improvement initiatives and patient care reviews.
Requirements:
  • Master's degree in Nursing from an accredited program.
  • Current Nurse Practitioner license in Texas.
  • Certification as a Family Nurse Practitioner (FNP) required.
  • Minimum of 2 years of clinical experience in a primary care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Commitment to patient-centered care and continuous learning.
  • Proficiency in electronic health record (EHR) systems.
