Job description

Hillcrest Rehabilitation and Healthcare Center located in Lower Burrell PA, is actively seeking an enthusiastic Medical Records Coordinator to join our caring team. We offer a nurturing home-like atmosphere with a capacity of 103 beds. 

  • Maintain resident medical records in accordance with HIPAA, CMS, Department of Health, and facility policies.

  • Assemble, organize, audit, and maintain resident medical records for admissions, transfers, discharges, and readmissions.

  • Ensure all resident documentation is complete, accurate, and filed promptly.

  • Monitor incomplete medical records and communicate deficiencies to the appropriate department.

  • Scan, upload, and maintain electronic medical records while ensuring document accuracy.

  • Process requests for medical records while maintaining resident confidentiality and following all applicable regulations.

  • Maintain medical record filing systems, logs, and record retention procedures.

  • Assist with physician order filing and document management.

  • Prepare records for state surveys, audits, insurance reviews, and legal requests as needed.

  • Perform routine chart audits and report findings to facility leadership.

  • Maintain admission, discharge, and census documentation as assigned.

  • Ensure all medical record forms comply with current regulatory requirements.

  • Work collaboratively with nursing, therapy, social services, admissions, and other departments.

  • Safeguard resident health information and maintain strict confidentiality.

  • Maintain knowledge of current federal and state regulations related to health information management.

  • Perform other duties as assigned by the Administrator or designee.
