Facility in Alcolu, SC seeking In-Home Primary Care NP locums coverageย Rates: $110/hrOpportunity Highlights
  • Contract Term: 13-week initial assignment with month-to-month extensions.
  • Guaranteed Pay: Paid for a full day regardless of patient volume (provided availability is logged in Salesforce).
  • Schedule Flexibility: * Full-Time: 40 hours/week (Monโ€“Fri, 8 amโ€“5 pm).
    • Part-Time: 20 hours/week options available.
  • No Call/Weekends: Enjoy a predictable daytime schedule with no on-call requirements.
  • Clinical Mix: 70% Telehealth / 30% In-Home
  • Support: Mileage is reimbursed at the IRS standard rate for home visits.
Clinical Role & ResponsibilitiesThis is an assessment-only role. You will focus on identifying clinical needs and chronic conditions to ensure patients receive proper follow-up from their PCPs.
  • Daily Volume: Conduct 5โ€“6 comprehensive assessments per day.
  • Focus Areas: Perform annual risk assessments, chronic condition reviews, and social determinant screenings.
  • Diagnostics: Perform A1C testing when applicable; no other invasive screenings are required.
  • Care Coordination: Identify and escalate urgent findings directly to the patientโ€™s Primary Care Provider.
  • Prescribing: None. This role does not require prescribing or active treatment plans.
Provider Requirements
  • Board Certification: BC Family Nurse Practitioner (FNP) (Required).
  • Licensure: Active South Carolina (SC) Medical License (Required).

  • Logistics: Must be comfortable with both telehealth and traveling within Richland County for home visits.
