GENISCi Health and Wellness (GENISCi) is a Virginia-licensed home care organization providing coordinated in-home skilled care, clinician-led healthcare support, care coordination, whole-health services, and technology-enabled health and wellness solutions.

Our care model brings together licensed clinical oversight, qualified in-home care personnel, client and family engagement, and responsible use of health information to support safety, function, independence, and continuity of care.

Position Summary

GENISCi Health and Wellness is seeking a motivated, professional, and relationship-focused Community Outreach & Care Coordinator to support community engagement, referral development, client and partner communications, marketing initiatives, and coordination of GENISCi's Health and Wellness services.

This is a part-time, on-site position based in Leesburg, Virginia. The employee is expected to work from the GENISCi office during established scheduled hours. Local community outreach, meetings, healthcare facility visits, networking events, and other approved off-site activities may be required as part of the position.

The ideal candidate will have experience in healthcare, home care, home health, community outreach, marketing, business development, care coordination, or a related field and will be comfortable communicating with healthcare professionals, community organizations, prospective clients, and families.

Essential Job Responsibilities

Community Outreach & Referral Development

  • Develop and maintain professional relationships with healthcare and community referral sources, including physician practices, hospitals, rehabilitation facilities, senior living communities, skilled nursing facilities, community organizations, and other healthcare partners.
  • Conduct outreach to introduce GENISCi Health and Wellness and educate community partners about the organization's available services.
  • Identify prospective referral relationships and new opportunities for community partnerships.
  • Follow up with referral sources and prospective community partners in a professional and timely manner.
  • Represent GENISCi at approved community events, networking opportunities, health fairs, educational programs, and professional meetings.
  • Assist with scheduling and coordinating meetings, presentations, and outreach visits with healthcare and community organizations.
  • Maintain ongoing communication with established referral and community partners.

Client and Care Coordination Support

  • Respond to prospective client and family inquiries and appropriately route clinical or service-related questions to designated GENISCi personnel.
  • Support the client intake and onboarding process by gathering appropriate non-clinical information and coordinating next steps with the appropriate team members.
  • Assist with communication among prospective clients, families, referral partners, and GENISCi personnel.
  • Help coordinate appointments, consultations, assessments, follow-up communications, and other client-related activities.
  • Maintain organized records of inquiries, referrals, follow-ups, and outreach activities.
  • Support continuity of communication throughout the client service process.
  • Escalate clinical questions, concerns, or changes in client condition to appropriately licensed clinical personnel and refrain from providing clinical advice outside the scope of the position.

Marketing & Community Engagement

  • Support marketing and community awareness initiatives for GENISCi Health and Wellness.
  • Assist in developing and distributing approved educational, outreach, and marketing materials.
  • Support GENISCi's digital presence through approved social media, community engagement, and awareness activities.
  • Assist with creating content for community outreach, educational initiatives, social media, and other approved communication channels.
  • Help promote awareness of GENISCi's skilled care, clinician-led care coordination, whole-health, health advisory, and wellness services.
  • Support reputation management activities, including appropriate engagement with Google Business, social media platforms, and community channels.
  • Ensure marketing and outreach communications accurately represent GENISCi's services and comply with company policies and applicable requirements.

Administrative & Office Support

Maintain accurate and timely documentation of outreach activities, referral contacts, prospective clients, follow-up activities, and outcomes.

Maintain organized electronic and physical records as required.

Assist with preparing outreach reports, contact lists, referral tracking, presentations, and other administrative materials.

Participate in scheduled staff meetings, operational meetings, training sessions, and other required company activities.

Coordinate assigned tasks and schedules with management and other GENISCi personnel.

Perform additional administrative, outreach, marketing, and care of coordination support duties as assigned.

Qualifications

  • Experience in healthcare, home care, home health, senior care, community outreach, marketing, business development, care coordination, or a related field preferred.
  • Knowledge of the Northern Virginia healthcare and community-services market is preferred.
  • Strong verbal and written communication skills.
  • Strong interpersonal, presentation, networking, and relationship-building abilities.
  • Professional and comfortable communicating with physicians, healthcare professionals, community organizations, clients, families, and business partners.
  • Strong organizational skills and attention to detail.
  • Ability to manage multiple priorities and consistently follow through on assigned responsibilities.
  • Ability to work collaboratively while also completing assigned responsibilities independently.
  • Proficiency with Microsoft Office, Google Workspace, CRM or client-management platforms, and social media tools.
  • Bachelor's degree in marketing, communications, healthcare administration, business, public health, or related field preferred.

  • Must maintain confidentiality of client, employee, business, and protected health information in accordance with GENISCi policies and applicable laws and regulations.
  • Must comply with GENISCi policies, procedures, standards of conduct, and confidentiality requirements.
  • Must successfully complete required background screening and other pre-employment requirements.
  • Must satisfy applicable health screening and personnel requirements established by GENISCi and Virginia regulatory requirements.
  • Must complete required orientation, training, and ongoing education applicable to the position.
