Job Title: Diabetes Educator
Employment Type: Part-time/20 hours weekly
Shift: Day Shift
Location: Silver Spring, Maryland
Position Purpose:
Serve as Diabetes Educator for Community Health's Diabetes Education Programs through planning, implementing, and evaluating the delivery and quality of education provided to both health center patients and the broader community. Serves as a mentor, role model, consultant, facilitator and educator.
Diabetes Educator supports the department in fulfilling the following Community Health Division goals: decrease avoidable hospital utilization; improve overall health through strategic partnerships, particularly with those addressing social determinants of health, and strengthen integration and linkages by wrapping community health programs and services around defined populations, services of the Holy Cross Health Network and both hospitals.
Supports the Mission of Trinity Health and Holy Cross Health.
Minimum education/licensure/certification required:
  • Bachelor's degree in health-education related field.
  • Master's Degree in clinical specialty area or related field preferred.
  • CPR certified within probationary period.
  • RN, RD or pharmacist with training and minimum of 2 years' experience pertinent to Diabetes Self- Management Education and Support.
  • Certified Diabetes Care Educator Specialist (CDCES) or Board- Certified Advance Diabetes Management (BC-ADM) certification preferred.

Minimum Knowledge, Skills, & Abilities Required:
  • Experience with program planning and evaluation. Ability to evaluate teaching when appropriate (i.e., pre- and post-test, repeat demonstration, clinical measures).
  • Specialized clinical knowledge in diabetes and behavioral change principles.
  • Excellent interpersonal and customer service skills.
  • Excellent oral and written communication skills. Bi-lingual in English/Spanish preferred.
  • Computer literate with proficiency in Microsoft Office software (Microsoft Word, Excel, PowerPoint) Duplicate.
  • Able to prioritize responsibilities and adapt to changing environments as necessary.
  • Maintains high level of confidentiality.

Pay Range: $35.80 - $51.82. Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
